Dear Service Providers,
The Add New Service Agreement button will be available on Friday, May 1, 2026 in the DVR Service Provider Portal on the Agreement Page (image included below).
Agreements for the 2026-2028 term must be submitted to DVR via the portal by Friday, May 15, 2026.
Agreements submitted to DVR after May 15, 2026 may not be processed until August 2026. In these cases, new DVR referrals may not be issued until the Agreement is approved.
