DVR Service Provider Portal Checklist Update

2026-2028 Agreement

Due by April 15, 2026:

• Review All Fields on the Details Tab and update as needed.
• Update/Inactivate Service Locations (Service Locations Tab).
• Update/Review All Active Personnel Profiles (Personnel Tab):
  • Locations Served.
  • Services Provided to DVR Consumers Service/County Grid.
  • Employment Type (FT/PT).
  • DVR Statewide Service Provider Training.
  • DVR IT Security Date (Completion date must be in 2026).
  • Criminal Background Check (CBC) Dates (completion date must be within six (6) months preceding the signing of the 2026 Agreement).
  • Ethics Course Date (Completion date must be within five (5) years).
  • Specialized Training, Certifications, and/or Credential(s).
• Update/Review All Subcontractor Profiles (if applicable) (Subcontractor Tab):
  • Services Provided to DVR Consumers.
  • Counties Served.
  • DVR Statewide Service Provider Training.
  • DVR IT Security Date (Completion date must be in 2026)
  • Criminal Background Check (CBC) Dates (completion date must be within six (6) months preceding the signing of the 2026 Agreement).
  • Ethics Course Date (Completion date must be within five (5) years).
• Enter Effective End Dates for inactive Personnel and Subcontractors: Upon entry, DVR is notified about Personnel with any level of Web Access that leave agency.

Due by May 15, 2026:

• Add new service agreement (available May 1, 2026 on the Agreement Page.)
• Review and complete Agreement Certification Forms.
• Review and complete Agreement Signature Form.
• Submit New Service Agreement
